Club Handbook

This handbook serves as a comprehensive guide for Mesa Community College clubs and organizations, outlining expectations, policies, and resources to support successful student involvement.

Statement of Club Activity Form

This form is used to register student clubs and organizations for the 2025–2026 academic year and to submit updates to existing club information. Clubs should complete this form to report changes to officers, advisors, meeting locations, or meeting dates and times. Keeping this information current ensures your club remains in good standing and allows Student Life & Leadership to provide accurate support and communication.

Club Roster Submission Form

This form is used to collect official club rosters for each student club and organization. All clubs are required to submit a complete roster twice per semester.

New Club Application

This application is for students interested in forming a new club or student organization and outlines the required steps for approval. To apply, clubs must have a faculty or staff advisor (with at least one full-time employee), a completed club constitution, and a minimum of 25 signatures with verified student ID numbers from currently enrolled MCC students. These signatures must be collected from the student body before submitting the application and uploaded as part of the required documentation.

Equipment Request Form

This form is used by clubs and student organizations only to request equipment from Student Life & Leadership for approved meetings, programs, or events. Requests must be submitted at least one week in advance to allow for processing and availability. Equipment is reserved on a first-come, first-served basis. Sandwich Board Request

This form is used by student clubs and organizations to request sandwich board placement through Student Life & Leadership for club-related business, including events and meetings. Requests must be submitted at least one week in advance, posters must include the MCC logo, and a maximum of four one-sided boards may be requested per club at a time. Sandwich boards are displayed for up to two weeks and are removed once the event or meeting has passed; directional signage is not provided. Clubs may bring printed posters from the Copy Center or upload their flyer through this form for printing assistance. For questions or assistance, please contact Allison Nguyen at [email protected]

Photo Release Form

This form grants permission for Mesa Community College to photograph, record, and use images or video of participants during club activities, events, and programs. These images may be used for educational, promotional, or marketing purposes by the college. Signing this form helps Student Life & Leadership share and highlight student involvement across campus. Participation in certain activities may require a completed photo release form.

Fundraising Form

This form is required for registered clubs and organizations planning to fundraise on campus and helps ensure events follow MCCCD and Mesa Community College guidelines. It collects key details about the fundraiser, cash handling, advisor responsibilities, and account information to support proper planning and financial accountability. Please submit the form 2–4 weeks in advance and update it after the event with the required Cashier Services receipt. Full instructions and requirements are included within the form for reference.
