Application for Departmental Credit by Evaluation form

Purpose

Departmental Credit By Evaluation is administered in accordance with AR 2.2.4. Students may apply for Credit By Evaluation in certain courses by obtaining an Application for Departmental Credit by Evaluation form. Some academic departments may have additional requirements that must be met before credit may be granted through Departmental Credit By Evaluation.

When credit is granted a notation of "Credit By Evaluation," and the number of credits will appear on the student's transcript. These credits are not used in computing the grade point average.

Students may not request:

  • To evaluate a course a second time;
  • To evaluate a course while currently enrolled in the course;
  • To establish credit in a previously completed course; and
  • To establish credit for a lower level of a course in which credit has been received.

Credit by evaluation is transferable within the Maricopa Community Colleges, but is not necessarily transferable to other Colleges and Universities.

Procedure

Step 1:

Student contacts the appropriate Academic Department to determine if Credit by Evaluation for the course is available, then completes applicable fields on the Application for Departmental Credit by Evaluation form to include:

  • Signature to certify that they understand the requirements and conditions for Departmental Credit by Evaluation
  • Completes the Student Information and Course Evaluation fields.
  • Obtains written administrative approvals as specified on the form.


Step 2:

The Faculty, Program Lead, or Department Chair submits the Application for Departmental Credit by Evaluation form along with documentation and rationale to the Chair who has oversight for the Program of Study.

Step 3:

The Chair reviews the request and sends it to the Dean of Instruction who has oversight for the Program of Study.

Step 4:

Requests must be approved by the Dean for which the student's program is offered. If the request is approved, the Dean submits the request to the Transcript team for final review and processing. The decision of the Dean is final.

End of Process
