Student Care Referral

When faculty or staff have a concern about a student for any reason, please reach out to the student to attain further information. Ask the student to meet with you during office hours or send the student an email. Depending on the student’s response, or if the student does not respond, please submit a Care Report. Student Care Reports may be submitted for the following situations:

  • Academic Concerns
    • Difficulty communicating or comprehending ideas
    • ESL
    • Difficulty with technology
    • Difficulty comprehending instructions
    • Difficulty understanding instructors directions
    • Student not engage in classroom activities
    • Lack of appropriate resources for classroom engagement
    • Student has need for tutoring resources
    • Other
  • Financial Concern
    • Financial aid
    • Transportation Assistance
    • Assistance with housing
    • Assistance with food
    • Job opportunities
    • Financial hardship
    • Other
  • Personal Concern
    • Student exhibits changes in appearance / behavior that warrants concern
    • Faculty or staff has knowledge of personal concern
    • Social & Behavior changes
    • Substance abuse
    • Mental health concern
    • Medical concern
