Required MCC Paperwork
All students must complete the Assumption of Risk and Enrollment form before starting their service-learning hours.
This form indicates that community-based learning may involve risks and confirms that the student has been approved to serve at the site, allows the student and supervisor to outline responsibilities, and includes conduct policies for the student.
Validation of Hours
Some instructors may require that you submit your time log to our office. Please enroll in our Canvas course in order to upload your Contact and Time log using the "Assignments" or "Modules" menu option. If you have any difficulties, please email a copy to [email protected].

Certificate
Students who complete at least the minimum service-learning requirement for their course are eligible to receive a certificate. To receive a certificate, students must complete all required paperwork and hours at an approved agency.
Independent Module
For students in a stand alone service-learning course. Please contact [email protected] if you are interested in enrolling in an independent module.
