Instructional Grievance Process Form
This form is for a student who feels that they have been treated unfairly or unjustly by a faculty member with regard to an academic process such as grading, testing or assignments. According to the Instructional Grievance Process, the student shall discuss the issue first with the faculty member involved.
STEP 1: Please read the S-6 Instructional Grievance Process & review your class syllabus to ensure you understand your rights and responsibilities.

STEP 2: Before you begin, make sure you know of which department your class/instructor is a part. You can find this on the instructor's Contact page.
