B & G Requests
Please follow these directions carefully so as not to delay the approval process for your project.
- Complete the online pdf form.
- You cannot save the data on the form. You must print the form to retain your information.
- After completing the form - print at least 2 copies. Keep a copy for yourself.
- Deliver 1 copy of the completed pdf form to your immediate Department Chair, Manager or Director.
- If approved the form will be forwarded to the Dean of your specific area
- If approved the form will be forwarded to the Vice-President of your specific area for final approval.
You will be notified if your project is approved or denied.
If you are unable to complete the pdf form online - please print this version and complete by hand then continue from Step 3.
