B & G Requests

Please follow these directions carefully so as not to delay the approval process for your project.

  1. Complete the online pdf form.
  2. You cannot save the data on the form. You must print the form to retain your information.
  3. After completing the form - print at least 2 copies. Keep a copy for yourself.
  4. Deliver 1 copy of the completed pdf form to your immediate Department Chair, Manager or Director.
  5. If approved the form will be forwarded to the Dean of your specific area
  6. If approved the form will be forwarded to the Vice-President of your specific area for final approval.

You will be notified if your project is approved or denied.

If you are unable to complete the pdf form online - please print this version and complete by hand then continue from Step 3.
