To request Disability Services, you must complete the Student Contract and the Student Contact forms and submit them to DRS prior to requesting accomodations. No services will be provided without these forms.

Request a Notetaker for your class.
Request a test accommodation.
Report an Interpreter no-show.
Report a Student no-show.
Provide Disability Resources & Services your contact information for interpreting services.
Acknowledgement that you have read the Deaf and Hard of Hearing Student Handbook and agree to the terms and conditions.
